First Time Applicants
All applicants must appear in person before a passport agent and submit a completed passport application. Application requirements include:
- Proof of U.S. citizenship
- Valid driver's license or state issued identification card (more information may be needed for state issued identification cards and out of state cards)
- Social Security number
- One passport specific photo; which can be taken at our facility for $15
- Separate payment for each application (check or money order payable to US Dept. of State), this will be sent in with the application
- Payment for the processing fee and photo (cash, credit card, or check/money order payable to Blue Earth County)
Passport Applications for Minors
Minors, under 18, that do not possess their own identification will need a parent to present theirs.
Minors, 15 years and under, must have both parents' consent to apply for a passport.
Download and fill out a passport application (DS-11) before visiting the Passport Agent located on the 2nd floor of the Government Center.
